A lead form turns a stranger's answers into a contact and a lead under Leads. It needs an email question and a name question, and it takes submissions only while it reads Active. Build it in the form builder, preview it, then switch it on.
Before you start
The form's Event type question offers your own event types, so name them first in Event types. Name your event types shows how.
Your role needs permission to change forms. Without it, the Forms page reads "View only. Your role can't change forms. Ask an admin." By default Operations and Accountant seats can't open the page.
Make the form
- Open Forms. It's under Settings, then Documents.
- Click New form, then choose Lead capture. EventSage makes a form called "New lead form", switched off, and opens it in the form builder.
- Type your own name for the form in the Form name box at the top. Only your team sees this name.
- The banner reads "This lead form can't receive submissions yet." Click Add email field, then Add name field. Each adds a required question the form cannot work without.
Add your questions and switch it on
- In the Elements tab on the left, click a tile for each question you want to add. Ask for the event date and the guest count. Each extra question loses a few people.
- Click a question to edit it. Type the Label your lead will read, add Help text if the question needs it, and turn Required on or off. Changes save automatically.
- Open The answer goes to and pick a Smart field, such as Event date, Guest count or Event type. The answer then fills the lead's record. A question with no smart field is kept with the submission only. Picking Event type turns the question into a dropdown of your own event types.
- On the Event type question, open Choices. Under Show on this form, clear the box beside any event type you don't want offered. At least one must stay checked.
- Click Preview. The form opens in a new tab as a lead sees it.
- At the top, switch the toggle from Inactive to Active. It saves the moment you flip it.
To share the form, click Share, then Copy URL. To put it on your own site, see Put your lead form on your website.
Test it with your own email
Fill the form in with an email address of your own, then look for the lead under Leads. A test lead is a real lead. If the form sends an email when someone submits, that email goes to the address you typed.
Something look off?
Work through each check in order. Most fixes take under a minute.
The form will not switch on
- 1 · Read the banner. The toggle refuses to turn on while the form lacks an email question or a name question tied to the client. Click Add email field or Add name field.
- 2 · Look for a warning above the form. Two questions cannot share one smart field, and autosave pauses until you fix it.
Nothing arrives
- 1 · Check the toggle reads Active. An inactive lead form refuses every submission.
- 2 · Check the banner is gone. A form without an email question and a name question tied to the client rejects every submission.
